Completing Your Registration in Pennsylvania: Required Forms & Common Issues

To finalize your child’s enrollment, all required forms must be completed and submitted. Below is a list of what’s needed, along with tips to avoid the most common registration delays.
Required Forms
✅Emergency Contact Form
All lines must be filled in. If you prefer not to share certain details, write N/A (do not leave it blank).
The phrase "Same as above" cannot be used—each section must be completed.
Authorized pickup contacts must include name, address, and phone number.
The bottom signature box must be signed. (You may write N/A in the “wading and swimming” section if not applicable.)
✅Child Health Assessment / Physical Form
A school health form is acceptable in place of the childcare form.
If using the childcare form, the parent must sign beneath the top information box.
✅Immunization Records
Upload your child’s immunization record.
Use the shot record tracker provided to ensure all vaccines are documented.
✅Parent Agreement
Must be signed and list who may pick up your child.
Include your drop-off and pickup times.
✅Program Agreements & Policies
Parent Handbook Acknowledgment that includes Discipline Policy Agreement and Tuition/Payment Agreement
✅Parent Letter
Fill in the names of individuals authorized to pick up your child.
Signature required.
✅Medication Authorization Form (if applicable)
Only needed if medication will be administered during program hours (e.g., EpiPen, inhaler).
✅IEP (if applicable)
If your child has an IEP, it must be provided so staff can follow it.
Common Things that cause delays
❗Missing or Incomplete Health Assessments
❗Incomplete Emergency Contact Info
Missing phone numbers or addresses.
Missing authorized pickup details.
❗Immunization Records Not Uploaded
Families often assume this is included with the health form—upload separately.
❗Skipped Electronic Signatures
All required signature fields in the portal must be completed.
❗Policy Acknowledgements Not Checked
Registration cannot proceed until all policy boxes are checked.
❗Duplicate or Conflicting Registrations
Avoid creating multiple entries—resume your existing application if already started.
